If lost in defensive use, Axon will replace the TASER device free of charge within one year if the following information is provided to Axon:

1. Official police report documenting the incident & citing use/loss of the TASER device in self-defense
2. Proof of purchase of the TASER product (receipt, purchase order or invoice)
3. Name, phone number and physical address to send replacement TASER device
